About the Role

Clarvida is seeking a Care Coordinor to provide Targeted Case Management (TCM) and Behavioral Health Home (BHH) services to children and adults with behavioral or mental health diagnoses. In this role, you will meet with clients and families in their homes to assess strengths and emotional, behavioral, and physical needs, develop individualized trement plans, and connect clients with approprie community-based resources.

You will engage clients and families regularly to support progress toward goals and collabore with educional, vocional, medical, and behavioral health providers to deliver coordined, wraparound services.

Responsibilities

● Provide Targeted Case Management (TCM) and Behavioral Health Home (BHH) services
● Meet with clients and families in home and community settings
● Assess client strengths, needs, and barriers to care
● Develop and maintain individualized trement and service plans
● Link clients to community-based resources and supports
● Coordine care with medical, behavioral health, educional, and vocional providers
● Engage clients and families regularly to support progress and outcomes
● Maintain accure, timely, and compliant documention

Requirements

● Bachelor’s Degree in any field

● MHRT-c Preferred- Eligibility Required

● Valid Driver’s License and active automobile insurance
● Ability to work independently in the community while collaboring with a multidisciplinary team
● Strong communicion, organizional, and documention skills
